Case Management
Case Management is a key service offered to all clients in each of our shelter, transitional housing, A & D Treatment, and Multicultural Assistance programs.
Case management is a client-centered, goal-oriented process for assessing clients' needs and helping them obtain the services needed to overcome barriers to housing and employment, and helping clients achieve long-term self-sufficiency. We employ a highly effective case management system linking services and resources to client needs and goals. Case managers build a relationship of mutual trust and respect with the client. Our experience has shown us that this relationship makes a critical difference in client success.
In the transitional housing programs, the Case Manager visits the family each week in their home, and provides additional on-going contact and support.
Follow-up services and support are provided to FIT clients for up to 18 months after the family has graduated from the program.
